How to access the service
Referrals can be made by professionals, family members (as long as they have permission) or the individual themselves. This can be done by completing one of our referral forms or by contacting 01482 6200229 / firstname.lastname@example.org
In most cases, the first port of call to accessing our service would be to attend one of our drop-in information sessions. These sessions take place on the 1st Tuesday of every month (3pm – 5pm) and on the 3rd Thursday of every month (10am – 12pm). These sessions allow potential service users the opportunity to find out more about the services we provide, in an informal setting. There is no need to book on to these sessions and people can stay as long as they like.
Following this. potential service users will be asked to attend an Initial Consultation, so we can find out a bit more about them. The Initial Consultation is a non-intrusive, fact finding exercise to enable us to understand the service user, so we can best support their needs.
From here we devise a unique recovery plan for each person and identify any personal goals or aspirations they wish to achieve. Each support plan is reviewed every 3 months to celebrate achievements/progress made and to highlight any new or continued support needs.
